End-of-Life Journeys may include one-on-one and family visits, video calls, phone calls, and individual or group educational sessions. Services are tailored to your needs.

Advance Care Planning

Advance planning entails reflecting on and sharing medical and caregiving instructions, and does not require a diagnosis or prognosis. Preparing the appropriate directives helps ensure that your wishes are honored and guides decision-makers in understanding your values and priorities.

Life review involves recollection and evaluation of positive and negative memories. It is a natural part of facing the end of life, to assess successes and failures and find meaning and wisdom. Legacy projects honor and memorialize someone’s life to preserve the past and inspire the future.
